The LUT Supplemental Benefits Fund serves the members of the LUT by planning for and helping with the administration of a variety of important benefits such as vision care, dental care, and, most recently, the establishment of a comprehensive legal services benefit.
The Board of Trustees of the Supplemental Benefits Fund closely monitors Fund benefits utilization and costs and meets regularly throughout the school year to consider changes to the Fund that will serve the best interests of LUT members.
Recently, the Board of Trustees looked carefully at our dental plan and voted to make considerable improvements to our coverage in many areas.
- Plan Year Maximum (July 1-June 30): Effective with all covered dental services rendered on or after July 1, 2017, the plan year maximum will be increased from $1,500 to $1,750 for each covered person.
- Schedule of Plan Allowance Increases: Effective April 15, 2017, the Dental Schedule of Plan Allowances (reimbursement to network dentists and/or reimbursement to members who visit an out of network dentist) has been increased for many services.
- Co-Payment Decreases: Effective April 15, 2017, several in-network copayments for covered services, such as crowns, root canal, dentures and bridges, have been decreased.
- NEW BENEFIT - Dental Implants: Effective for dental implant procedures commencing on or after April 15, 2017, the Fund will pay up to the scheduled allowance for covered implant services, per covered person, up to a maximum of two (2) implants per mouth. This maximum is not included in the yearly dental maximum of $1,750. It is a separate maximum per lifetime – no more than two (2) implants per covered person per lifetime.
Important note: Reimbursement for dental implants will only be made under the applicable American Dental Association (ADA) procedure codes in the 6000 series, or as otherwise established by the ADA. Reimbursement will be limited to once, per tooth, per lifetime. Pre-treatment authorization is required before benefits are payable for dental implants.
Members are strongly encouraged to refer to the Schedule of Plan Allowances for a listing of Covered Services and Co-Payments when utilizing the services of a participating provider.
