Lawyers from Mirkin and Gordon, P.C., are currently working remotely.  The firm is fully functional at this time.
 
Lawyers are available to arrange video and/or telephone conferences and conduct new client consultations and intakes remotely, with any required paperwork circulated via scanned documents or fax.

Vision Benefits:
 
PLEASE NOTE: Effective 1/1/14, vision care benefits will no longer be payable for services rendered by an ophthalmologist.
 
Benefit payable no more than once every twenty four (24) months.
  • $175 Member

There are three options in the vision benefit:
  1. The GVS plan with many participating stores.

  2. Ultra Vision – one location in Levittown.

  3. Reimbursement for your own vision store.
    Download and print the Vision Reimbursement Form
    Return the form and your original receipt to the
    LUT office.  You have 90 days from the date of service to submit for reimbursement.
 
Dental Benefits:
 
Dental Insurance is S.I.D.S. At participating providers there are no “out of pocket” expenses on routine procedures.
  • $1750 max per person per year.

Health Insurance:
 
EFFECTIVE 1/1/14 Major Medical Deductible Reimbursement Benefit is discontinued.
 
Is provided by the district. The member pays a percentage as per the contract.(payment is provided to those who waive the health insurance.)
